Job Description
A facility located near Richmond VA is seeking a Director of Case Management!
Pay: $95000$105000/ Annually!
Director of Case Management Opportunity:
- Fulltime directhire opportunity
- Monday Friday
- Day Shift
- Opportunities for tuition reimbursement and continuing education
Director of Case Management Responsibilities:
- Director of Case Management helps collaborate with clients and families to set realistic and achievable goals
- Director of Case Management has 4 Direct Reports underneath them
- Advocate for clients rights and needs to ensure they receive the support and services they require
Case Management Director Qualifications:
- BSN or BSW
- Must be either a Licensed Registered Nurse or Licensed Social Worker
- 2 years of management experience
- 3 years of hospitalbased Case Management experience including Utilization Review and Discharge Planning experience
- 2 years of rehab experience preferred
- Current CCM or ACMTM certification is preferred
