Location:Manhattan NY
Responsibilities:
The Substance Use Disorder (SUD) Care Management Program is a program that works with high utilizers through early outreach and engagement transitional care comprehensive assessment and care planning and post program support. The program provides 9month Integrated Care Management for Members with Substance Use Disorders. The program is looking to recruit candidates who are specialized and have experience with the SUD population to apply for the following position.
Qualifications
Licenses and Certifications:
Current registration to practice as a Licensed Social Worker in New York State preferred. Valid drivers license preferred. Credentialed Alcoholism and Substance Abuse Counselor (CASAC) required.
Education:
Bachelors Degree in Human Services or related field or the equivalent work experience required. Masters Degree in Social Work preferred.
Experience:
Minimum of one year of training and/or experience in integrated mental health and substance abuse treatment required. Demonstrated skill or knowledge in substance abuse assessment and diagnosis principles and practices of harm reduction and application of stagewise treatment and motivational interviewing strategies required. Bilingual skills may be required as determined by operational needs. A field based position.
Compensation:$22.49 $28.12 Hourly
