Job Description

JOB SCOPE:This is a direct service position requiring specific knowledge of mental health and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires the desire to work as part of a team.

ESSENTIAL FUNCTIONS:

  • Participate in morning meetings and staff meetings.
  • Evaluate need for psychotropic medication; prescribes medications as needed educates clients and relatives about these medications and their effects reviews and adjusts dose level as needed and documents all medications in consumers electronic chart.
  • Conduct visits in the community in accordance with NYS Office of Mental Health (OMH) licensing standards.
  • Coordinate consumer access to medications with the RN and LPN nurses including administering injectable medications when needed.
  • Order routine blood work and/or toxicology testing as needed.

Salary:$150025 Annually

Scheduled Shift:MondayFriday 9AM to 5PM


EDUCATION AND EXPERIENCE:

  • Masters degree in advanced nursing from an accredited Nurse Practitioner Program; Current license in NYS as a Psychiatric Nurse Practitioner; NYS license as a Registered Nurse.
  • Minimum of three 3 years working with people with serious mental health conditions cooccurring substance use disorders and homelessness.

SKILLS AND KNOWLEDGE:

  • Excellent communication skills and ability to interact with all clinical and administrative staff other professional staff and agencies clients and families.
  • Experience understanding various evidencebased models and theories pertaining to psychoeducation motivational interviewing serious mental illness and recovery.
  • Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
  • Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
  • Ensure progress notes are completed within 24hours of the delivery of the service and are available to be read in the daily organizational meeting.
