Job Description

Shift and Status

Full time 8:00 a.m. to 6:30 p.m. variable days and every fourth weekend.

Essentially remote will be out in the field visiting homes.

Job Summary

As a continuing care LPN with Corewell Health providing topnotch patient care is paramount. Under the direction of an RN midlevel Provider or Physician youll be responsible for assessing planning implementing and evaluating patient care. Youll be a vital part of our team making a real difference in the lives of our patients and their families.

Essential Functions

  • Collects and monitors assessment data and reports physiological psychological sociocultural and spiritual needs of patients and families in coordination with and as delineated by the RN/Midlevel Provider/Physician.
  • Provides direct patient care under the direction of the RN/Midlevel Provider/Physician consistent with policies and procedures and unit based routines.
  • Informs patients and families about treatments and medications being received equipment being utilized and other related activities as appropriate in coordination with the RN/Midlevel Provider/Physician.
  • Documents care provided via computer/written patient record in a manner consistent with guidelines.
  • Greets and communicates with patients and families in a manner consistent with caring and respect. Communicates and collaborates with other members of the health care team in order to ensure continuity of care and coordination of service.
  • Cleans patients environment/work area/equipment and communicates any equipment malfunctions to appropriate department/unit personnel.
  • Documents patient chargeable items according to standards.
  • Participates in selected quality monitoring activities under the direction of the RN / Midlevel Provider / Physician. Demonstrates a high standard of moral and ethical behavior. Recognizes appropriate and ethical behaviors and identifies resources for resolution as needed. Advocates for privacy confidentiality and security of patient staff and organization data.

Qualifications

Required

  • High School Diploma or equivalent
  • Completion of an approved nursing program
  • Successful completion of competencybased orientation program
  • LICLicense Practical Nursing (LPN) STATEMI State of Michigan Upon Hire
  • LICDrivers License STATEMI State of Michigan NRS Home Care VNA and Hospice except Hanson Hospice Center Upon Hire
  • CRTBasic Life Support (BLS) AHA American Heart Association Corewell Health Continuing Care (except Hospice) including Regional Rehab and Nursing Centers Lakeland Continuing Care Caring Circle Hospice 90 Days required Or
  • CRTBasic Life Support (BLS) ARC American Red Cross Corewell Health Continuing Care (except Hospice) including Regional Rehab and Nursing Centers Lakeland Continuing Care Caring Circle Hospice 90 Days

Preferred

  • Associates degree or equivalent Graduate of an approved nursing program
  • Healthcare experience
  • CRT Brain Injury Specialist Certified (CBIS) ACBIS Academy of Certified Brain Injury Specialists Continuing Care Neuro Residential 2 years
