FunctionsVerify and document that all students comply with Vermont School Immunization a cumulative Permanent School Health Record on every matriculated vision hearing and other screenings as outlined in the Vermont School Nurse students who are referred by staff and/or present to the health officeObtain health and developmental historyReview of health recordsIdentify health findings which do not fall within the nursing interventions for acute and chronic health concernsDocument assessment data and nursing actions in the Electronic Health recordAdminister and document medications according to district policyCollaborate with parent(s) legal guardian(s) teachers guidance counselor social worker school psychologist administrators and community resources regarding student health needs/problems to assist the student educationally and enhance his/her well student and staff privacy/confidentiality as per HIPPA FERPA and district policyCollect document and analyze data regarding suspected child abuse and neglect and make formal referrals to appropriate nursing tasks responsibly per district policy to unlicensed and licensed staff staff members as knowledgeable of community health resources and refer students/families to these resources as as the Health Specialist in referrals and meetings to develop Individualized Education Plan (IEP) Section 504 of the Rehabilitation Act ofand Response to Intervention (RTI) plans as needed and act as case manager when and implement Individual Healthcare Plans (IHP) and Emergency Care Plans as and analyze data to identify trends in illnesses and injuries and recommend appropriate mitigation in reviewing and developing health related district policies and with district and other school nurses to analyze and resolve student health with other school personnel as appropriate regarding student health statistical reports for the Vermont Department of Health and other agencies as required. Provide inservice education to teachers and staff according to student health concerns staff requests emergency preparedness and other identified needs. Collaborate with teachers students and family members to present appropriate information to students regarding health issues as with and/or refer to state health agencies as input for preparation and implementation of individual schools Health Services nursing knowledge and skills through professional development workshops courses and other professional strengths and weaknesses through wellness and disease prevention through public health efforts such as bulletin boards newsletters and other opportunitiesWorking Conditions: Maintain emotional control under stress work with frequent interruptions.
