Job Description
In this vital role you will help patients understand their rights and responsibilities and guide them in managing the physical mental emotional and financial demands of End Stage Renal Disease. This can include crisis counseling mental health services and connecting patients with community programs and resources.

- Must be comfortable working between two clinic facilities University Dialysis 550 University Blvd Indianapolis IN) and Franklin Dialysis 1140 W Jefferson St Franklin IN)
Requirements of the position:
- Masters degree in Social Work (MSW)required
- Licensed or registered in the state of Indiana as required by state regulations.
- Comfortable floating between two clinic facilities mileage compensation available.
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word Excel PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
- Dedication to caring for patients suffering from chronic kidney failure.

Salary/ Wage Range
Compensation for the role will depend on a number of factors including a candidates qualifications skills competencies and experience and may fall outside of the range shown.
