Job Title: Family Specialist ACT WellLIfe

Position Reports to Job Title: Team Leader

Position Summary:

A clinical staff member who in addition to performing routine team duties has lead responsibility for integrating family goals and services with the tasks of all team members and for providing family psycho education individually and in groups.

Essential Accountabilities:

1. Demonstrate adequate knowledge of current DSM an understanding of AXIS I diagnosis providing psychoed services in group individual and family settings (wellness; community integration; vocational services).
2. Work environment requires flexibility with on and off site work locations working as part of an interdisciplinary team and also individually.
3. Manages caseload in terms of:
   1. Schedules appointments in a timely manner.
   2. Addresses cancelled/failed appointments from both clinical and administrative perspectives.
   3. Is aware of and attains productivity expectations.
4. Completes documentation as required:
   1. Completes a treatment plan documenting goals/objectives that are behaviorally measurable. Supervisor and team psychiatrist must review and sign the Plan.
   2. Completes treatment plan reviews on a biannual basis (every six months). Reviews must be done within a six month period; cannot exceed the six month period even by one day.
   3. Each review must address goals/objectives in the treatment plan and be reviewed by Psychiatrist and assigned staff for Utilization Review.
   4. Documents all contacts in progress notes both facetoface and phone contacts. Notes are to relate to the goals/objectives on the treatment plan.
   5. Completes all correspondence in a timely manner with signature of Psychiatrist/NPP and supervisor noting approval.
   6. Provides direct services in the community where consumer is most comfortable (i.e. home work stores etc..
5. Assists in the planning developing and implementation of goals and training to further develop participants skills including but not limited to:
   1. Budgeting
   2. Grooming
   3. Nutritional planning
   4. use of public transportation
   5. use of community resources
   6. Shopping
   7. Housekeeping
   8. Physical health & development
   9. Adaptive behavior/independent living
   10. Nutrition
   11. Vocational development
   12. Social development

Qualifications:

• One year of training and/or experience in family assessment and intervention psychoeducation and/or other family support services including cognitivebehavioral strategies.
• Must hold a masters degree in social work or related human service field and valid drivers license.
• Experience working with behavioral health population preferred.
• Computer proficiency including the ability to use email navigate and enter all documentation into electronic medical record.
• Valid drivers license and vehicle required candidates are subject to regulatory background checks and must be willing to utilize own vehicle for home visits.
