Under the direct supervision of a pharmacist performs transition of care (TOC) related tasks such as collection and verification of allergy and medication history. The TOC liaison will communicate with patients families nurses physicians and other healthcare professionals to obtain accurate home medication lists upon hospital admission. Position is expected to work from the Emergency Department and perform Medication histories. There is not a current weekend rotation but in the future a q34 weekend rotation is expected.
