Overview
RN Nurse Navigator Clinical Admissions Liaison
Full time 40 hours weekly. Monday Friday 8:00am 4:30pm Weekends as needed
RN Navigator will cover a territory local travel required
Must have experience in an inpatient acute rehab admission liaison expeirence a plus

Know Your Rights: Nurse Navigator will be responsible for coordinating service and identified patients through a complete health system by assisting with access issues identifying resources providing educational material and developing relationships with service providers and community. The Nurse Navigator coordinates the care in a timely and efficient manner identifying and eliminating barriers to care. The Nurse Navigator is a consistent care coordinator throughout the continuum of care and assess the physical psychological and social needs of the patient enhancing patient outcomes increasing satisfaction and reducing cost of care. The Nurse Navigator is responsible for community education and screening as well as monitoring community outreach and promoting survivorship.
Qualifications
EDUCATION EXPERIENCE TRAINING
1. Current and valid state RN License.
2. Current BLS certificate upon hire and maintain current.
3. Bachelor of Science in Nursing (BSN) preferred.
4. Minimum of two 2 years of nursing experience.

A reasonable compensation estimate for this role which includes estimated wages benefits and other forms of compensation is $35.35 to $47.58. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire in which a wide range of factors will be considered including but not limited to skillset years of applicable experience education credentials and licensure.
