Responsibilities
- Must be willing to travel to/from all sites and offsite meetings. Must be open to offhour events and meetings with flexibility to attend on weekends as requested.
- Supervise site nursing managers and the Chronic Care Nurse Manager coordinating daily operations and expectations
- Serve as point of contact for the ACO and oversee Chronic Care Management
- Partner with nurse and site managers to lead all nursing services ensuring alignment with department philosophy policies and standards
- Ensure adequate RN LPN and MA staffing and competency across sites
- Promote fullscope practice for RNs LPNs and MAs supporting topoflicense care
- Pilot new nursing roles and care models to improve safety efficiency and patientcentered care
- Develop key performance indicators for staffing and performance metrics
- Design onboarding and ongoing education programs to support retention and professional growth using existing software
- Collaborate with leadership on clinical quality compliance safety access utilization and system improvement strategies
- Support regulatory readiness (Joint Commission NCQA DPH FTCA HRSA)
- Act on patient satisfaction data incident reports and access metrics to improve nursing care and wait times
- Lead annual employee flu clinic
- Contribute to the Nursing Department budget with medical and operational leadership
- Maintain infection control and occupational health policies with focus on COVID19 and contact tracing
- Participate in key organizational meetings including Primary Care Collaboration Medical Department meetings Quality Council ACO Population Health Credentialing and others
Requirements
- Active Massachusetts RN License
- 5 Years of Experience as an RN with 3 years of Experience in a Supervisory/ Management Role
- 3 Years of Experience working in Primary Care as an RN
- Bilingual proficiency in Spanish Portuguese or HaitianCreole and English is a plus but not required
- Masters Degree Preferred
- Excellent communication and interpersonal abilities
- Commitment to providing compassionate and patientcentered care
