Job Description
Hybrid Remote Role no call no weekends.
DaVita Integrated Kidney Care (IKC) is looking for a FullTime Nurse Practitioner (NP)to join us in Lincoln NE willing to travel Omaha NE.
As an IKC Nurse Practitioner (NP) you will support some of our most complex patients assisting them in navigating a challenging healthcare system. Youll focus on delivering holistic integrated care to improve the quality of life for our kidney patients. Youll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality consistent costeffective care.
Position Details
Location: HybridRemote. Work from home and Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.
Complete Comprehensive Health Assessments
Manage complex patients focused on reducing hospitalizations and improved health outcomes
Monday Friday schedule with the ability to accommodate patient and nephrology partners availability. Must be flexible.
FullTime selfmanaged schedule
No nights weekends and oncall!

Requirements
Passion for delivering care to patients with complex chronic illnesses
Minimum of two 2 years experience as a Nurse Practitioner (NP)
Current unrestricted NP license to practice in the state
Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
Intermediate computer skills and proficiency in MS Word Excel & Outlook required
Home Office High Speed Internet and Reliable Transportation Required.
Preferred Qualifications
Preferred experience working in Value Based Care or Population Health including documenting and coding from a MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.
5 years experience as a Family Nurse Practitioner (FNP) Primary Care Nurse Practitioner (NP) or experience in Internal Medicine Cardiology Nephrology or Case Management
Spanishspeaking preferred in some markets
Experience in managing chronic diseases (CHF diabetes hypertension diabetes)
Manage pop. Health (hospitalization)

Compensation for the role will depend on a number of factors including a candidates qualifications skills competencies and experience and may fall outside of the range shown.
