Organization:
SEBMFEast Bay Medical FoundationPosition Overview:
A core member of the collaborative care team including the patients medical provider psychiatric consultant and the larger primary care team or medical team. Responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patients medical provider and other mental health providers.Job Description:
EDUCATION:
- Masters: Degree in a Mental Health discipline
- OR Graduate of an accredited school of nursing
- OR Doctorate in Clinical Psychology
CERTIFICATION & LICENSURE:
- None if possesses a Masters degree in a Mental Health Discipline
- Or RN
- Or PSYPsychologist
TYPICAL EXPERIENCE:
5 years recent relevant experience
SKILLS AND KNOWLEDGE:
- Ability to assess the needs of the patient provide high quality integrative psychotherapy guided imagery and other evidenced based approaches as indicated.
- Knowledge of holistic assessment care plan development and outcomes tracking methods.
- Candidate must have desire and ability to work as an active member of an interdisciplinary practice team led by physicians.
- Required to have knowledge of the most recent Board of Behavioral Science requirements and adhere to them.

Pay Range is $55.86 to $73.73 / hourThe salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidates experience education skills licensure and certifications department equity training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
