We serve the residents of Hamilton County in more ways than you may realize!In this roleas the QRT Navigator at Hamilton County you will provide direct client service and case management for individuals and families impacted by substance use disorders. You will connect clients with appropriate treatment mental and physical health services housing resources and support systems while maintaining accurate records and collaborating with community partners to ensure seamless service delivery
Summary of Job Duties:
  • Client Support and Services Navigation: Implement strategies to guide clients through systems of care including prevention triage service linkage followup and family support.
  • Assessment and Case Management: Evaluate and prioritize client needs connecting them with appropriate treatment and resources to support longterm stability.
  • Record Keeping and Data Management:Maintain accurate and uptodate client records using the RMS System (Cordata) and the RedCap Harm Reduction Tracking System.
  • Community Collaboration and Resource Coordination: Build and maintain relationships with HC ARC partners and community organizations to enhance access to services.
  • Budget Management and Administrative Duties: Monitor expenses (e.g. parking and mileage cost incurred during work trips bus pass account Uber account etc..
  • Team Coordination and Supervision Support: Proactively use the shared calendar to document activities; participate in daily briefings and debriefs with the coresponder team.
  • Program Development and Barrier Reduction: Assist with identifying gaps and barriers in client services and community resources using datadriven practices
Minimum Qualifications:
  • Bachelors degree in social work sociology psychology or a related field.
  • One 1 or more years of experience in social services case management or a related capacity.
  • or equivalent combination of training and experience demonstrating knowledge of social services and case management practices.
Work Location and Hours
  • 138 E. Court St. Cincinnati Oh. 45202.
  • 40 Hour Work Week generally between core hours of 7:305:30
