JOB SUMMARY: The 1 to 1 Job Coach is focused on assisting adults with disabilities to achieve their personal outcomes. The most important job for the 1 to 1 Job Coach is to safeguard the individual they are assigned to personal rights and life choices as it relates to vocational supports.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Provides daily job coaching 1:1 support for the individual with consistent and timely followup identifying and resolving barriers and challenges in the community and employment site while assisting the individual in accomplishing identified employment goals.

  • Identifies and develops employment to include volunteer for the individual that will lead toward potential employment opportunities working closely under the direction of the assigned Employment Specialist.

  • Plans community vocational exploration activities.

  • Responsible for following the behavior plan (if applicable) and acts as lead during any behavioral crisis.

  • May provide personal care services such as toileting grooming meal setup and feeding as needed in a safe and timely manner while maintaining dignity and respect for the individual.

  • May provide instructional support through communication devices/techniques proven to enhance and/or promote the development of independent communication. The techniques and devices will be those that have been determined by the family and other professionals to work best (i.e. PCS IPAD applications sign language etc..

  • Assists and contributes in the development of the IP for the individual focusing on pre employment and job readiness tasks.

  • Maintains collaborative relationships and communication with other program staff referring agencies family members and support persons to ensure individuals needs are met in a manner that supports the vocational outcomes identified in the IP.

  • Maintains forms to track individuals progress in completing IP goals as well as maintaining other required documentation in a timely manner as required by funding source.

  • Reports all incidents according to the Department of Developmental Disabilities Services Incident Reporting policy. Participates in investigations and participate in the implementation of the plan of correction when indicated.

  • Performs other jobrelated duties as assigned.

EDUCATION and EXPERIENCE:

  • High school diploma is required. Verification of foreign equivalency required. Some college a plus.

  • One year work experience specific to working with people with behavior and/or challenging behaviors in the community.

  • Must have experience with employment supports/case management of individuals with Intellectual Disability and severe Developmental Disabilities.

SKILLS and COMPETENCIES:

  • Proficient in Microsoft Office Microsoft Outlook 360 and web browsing.
  • Possess a full valid local drivers license and maintain a clean driving record.
