QUALIFICATIONS:
- Bachelors degree in social work or related discipline or;
- Eight years experience working with clients in crisis and the strong ability to effectively resolve and cope with immediate crisis situations.
- Experience in working with households inclusive of children under the age of 18
- Extensive knowledge of community resources
- Excellent knowledge of and ability to use current Microsoft software applications including Word Excel PowerPoint and Publisher as well as email Adobe Acrobat the internet & Bowman Systems (WellSky and/or HMIS depending on corps or grant requirements)
- Excellent interpersonal telephone and communication skills both written and verbal
- Excellent detail organization and file management skills
- Demonstrated dependability maturity and initiative; must be able to maintain program/guest/agency confidentiality and treat each individual with dignity and respect.
- Ability to work alone and as a part of a team.
- Ability to maintain a selfmotivated and selfdirected schedule & address multiple clients in a timely manner
- Ability to work with people of diverse backgrounds and circumstances
- Indepth knowledge of the Homeless Management Information System.
- Bilingual (English/Spanish) strongly preferred
- Must possess a valid Arizona drivers license with a clean driving record & pass a motor vehicle records check.
- Must have reliable transportation.
- Must be able to pass a criminal background check

- Ability to remain compliant with The Salvation Army Protect the Mission standards & protocols
RESPONSIBILITIES:
- Interview clients by phone and in person to determine eligibility for emergency assistance
- Interview clients to determine their needs and refer them to the appropriate community agencies and services
- Develop an individual action plan and set goals for each client
- Provide ongoing case management/assessments for clients at 30 60 90 & 365 days.
- Provide referrals to clients on housing health benefits financial support and other available community and public services
- Network with outside (appropriate) agencies and build relationships with community partners in service to clients
- Compile monthly statistical report as required for the POH program
- Maintain positive working relationship with Salvation Army representatives volunteers and all agencies providing services to clients.
- Participate in meetings with the Divisional Pathway of Hope Coordinator
- Submit guarantees to various utility companies
- Work with landlords to collect and process information for the purpose of rental assistance in an accurate and timely manner
- Maintain client confidentiality within all aspects of duties relating to the collection and reporting of clientbased information
- Act as a liaison between clients organizations and utility agencies
- Complete required paperwork accurately and in a timely manner
- Create and update manuals or computer database client files while maintaining records required for follow up
- Encourage clients to utilize community opportunities resources and services
- Keep a current file and update a register of social services available to area residents
- Gain and maintain a comprehensive knowledge of WellSky Community Services
- Communicate within the Emergency Services/Case Management team for decision making
- Demonstrate superior customer service integrity & commitment to innovation efficiency & fiscally responsible activity
- Participate in all Pathway of Hope (POH) trainings including but not limited to Strengths Based Perspective Motivational Interviewing Trauma Informed Care/DeEscalation and Stages of Change Model
- Support clients spiritual growth by connecting them to the local corps officer and/or ministry leader for pastoral care as is possible.
- Raise client awareness of and possible participation in corps activities which include but is not limited to: summer camps vacation bible school adult bible studies after school programs troops music and other youth activities.
- Support relevant corps events by providing Pathway of Hope promotional materials information and client referrals when appropriate.
- In partnership with the corps officer create possible volunteer opportunities for corps officers soldiers and Advisory Organization members to participate in the program.

- Utilize POH steps in navigating the path to independence and stability.
- Focus on POH outcomes.
- Other duties as assigned by the supervisor as it relates to the position of POH Case Manager
