Job Description

  • Full time position based at Blackburn
  • Permanent Full time contract
  • Provide information advice and individual support to carers
Your new role

Our Carer Services Program delivers a range of support services to carers who provide unpaid care and support to a family member or friend who is frail due to age or has dementia a disability mental illness terminal illness or chronic health condition.

As an Assessment and Planning Officer you will utilise the Carers StarTM conversation tool to understand and assess the specific needs associated with the caring role and develop goal orientated care plans in conjunction with carers. You will assist with planning and coordinating an appropriate service response which may include shortterm case management respite carer coaching and counselling peer support and referral to other services where necessary.
