Partnership Resources Inc. a nonprofit disability services provider in the Twin Cities/West Metro area is seeking a Designated Coordinator to join our fantastic Leadership Team. The Designated Coordinator is handson leading a team of Direct Support Professionals at our day program in St. Louis Park.

The St. Louis Park site is a Day Support Services program that supports individuals who have developmental and intellectual disabilities in enhancing life skills to achieve goals selfadvocate and foster joyful living. We strive to create opportunities that align with the wants needs and interests of individuals served.

Job Duties

  • Provides handson support in the day program coordinating site staff and client services
  • Hires trains and supervises a team of 810 Direct Support Professionals
  • Works with the CoCoordinator and Site Team Leader to lead the team in developing planning and implementing high quality client programming focused on recreation skill building and community engagement. We offer a wide variety of activities for participants to choose from and partner with community organizations to offer specialized opportunities
  • Manages a caseload of 3040 individuals served: overseeing 245D documentation and personcentered outcomes mentoring staff on implementation and leading support team meetings
  • Oversees and enforces health & safety procedures at assigned location including medication administration lifting/transferring and environmental hazards

Compensation

  • Pay: $21 or $23 per hour depending on qualifications

Requirements

  • Must meet Designated Coordinator qualifications in accordance with 245D licensing requirements:
    • Experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children AND/OR postsecondary education in a field related to human services education or health that totals a minimum of 4 years
    • Candidates who do not meet this requirement as demonstrated by their resume will not be considered
  • Ability to learn understand and implement Chapter 245D licensing requirements
  • Must be able to lift and transfer clients adhering to the physical demands of thejoband safety requirements/policy
  • Ability to travel between office locations as needed including providingtransportation to clients as assigned
  • Must be able to pass a background study with the MN Department of Human Services
  • Must be authorized to work in the United States without a sponsor visa
