With over 400 employees in Southern Colorado you would be joining a mighty team of support and administrative staff case managers clinicians physicians nurses and others in our efforts to improve the health and wellbeing of our community.

Weve got a great opportunity for an onsite Clinician to join us at our integrated family care clinic in Grand Junction Co.

What Youll Be Doing

As the Clinician you would assess and provide justright therapies for our clients in need. You would coordinate care actively liaise with internal and external partners and serve as an important part of a multidisciplinary team. You would employ excellent communication and documentations skills on an ongoing basis to assure that clients receive the very best care.

Specifically the Clinician

  • Conducting intake assessments to include substance abuse/mental health/cooccurring assessments.
  • Providing individual and group therapy.
  • Providing case management services.
  • Keeping accurate complete and uptodate records with 100 compliance with Center standards.
  • Maintaining 63 productivity for billable services monthly.
  • Maintaining competency and proficiency with the electronic health record.
  • Working with community agencies; and
  • Performing other jobrelated responsibilities as assigned.
  • Will be required to cross train in other duties as allowable by licensure.

Physical requirements

  • Requires standing sitting driving and walking for prolonged periods of time.
  • Occasionally lifts and carries items up to 20 pounds.
  • Frequently exposed tostressful or emergentsituations.
  • Frequently operatesa computer and other office equipment to record health data.
  • Constantly communicates with clients and others.
  • Occasional exposure to communicable diseases and bodily fluids
  • Occasionally drives between Health Solutions facilities or for other assigned duties.
  • Requires occasional driving in adverse weather conditions.

Qualifications

What Were Looking ForThe MustHaves

  • Masters degree in social work clinical psychology or a closely related field
  • Current Colorado professional license (e.g. LPC LCSW LMFT) or license within three years
  • Experience working successfully with a multidisciplinary team especially in an integrated primary care setting.
  • Ability to work in a fastpaced primary integrated healthcare practice.
  • Experience providing clinical services to patients of all ages.
  • Experience and comfort working with a wide range of behavioral health conditions and presentations to include substance use disorders.
  • Experience working with severe and persistently mentally ill adults and experience with various behavioral health diagnoses including substance use.
  • Good communication skills both verbal and written.
  • Exceptional time management skills.
  • Flexibility and ability to easily adapt to change.
  • Welldeveloped documentation skills.
  • Computer proficiency.
  • Valid Colorado drivers license and access to reliable transportation during work hours.
  • CPR/First Aid
  • Pass background check

What Wed Like to See in YouThe NicetoHaves

  • Mental Health Assessment skills and familiarity with the DSM are preferred.
  • Certified or Licensed Addiction Counselor preferred.
  • Experience completing atrisk assessments and formulating appropriate treatments plans.
  • Spanish language skills
