Job Description
Direct Medics Healthcare Recruitment are seeking a compassionate and skilledBand 6 Mental Health Practitioner to join the Community Mental Health Team based in the Preston area. This is an exciting opportunity to screen incoming referrals from Primary care and undertakes initial assessments as well as providing ongoing support to Adults with Severe and enduring and complex mental health issues.
Contract is up to 3 months initially with full time hours required (MondayFriday 9am5pm). Salary from 35392 to 42618 a year per annum.
In this role you will:
As a Lead Practitioner you will coordinate care by working closely with people in there own homes or in the community to help them recover from their illness and improve their general quality of life.
Care planning medication management and physical health monitoring managing patient documents and all other general nursing duties.
Requirements
- Recognised Core Mental Health Professional Qualification professional Registration such as NMC SW England
- Post qualification experience within mental health services. With some of these to have been within acute mental health services.
- Demonstrable experience of providing teaching/training
- Clear understanding of the Mental Health Act Mental Capacity Act and Care Act and how this influences practice within the CMHT inpatient and HTT settings
- Full UK Drivers License and Access to own car
- Right to Work in the UK
