Under the direction of the Director of Pharmacy the pharmacist is responsible for the safe and appropriate use of pharmaceuticals throughout the hospital. Holds primary responsibility for interpreting medication orders obtaining preparing and dispensing medications and providing drug information.
Education
PharmD degree or Bachelors degree in Pharmacy from an accredited program.
Certification Registration & Licensure
Licensure required: NH Pharmacist license or reciprocity pending.
Board Certified Pharmacotherapy Specialist (BCPS) is desirable.
Experience
A Pharmacy residency is desirable.
Responsibilities
Identifies and resolves actual or potential medication related problems.
Takes ownership of pharmacy related issues and follows them to completion.
Provides accurate drug information to staff providers and the community.
Assist physicians and residents to develop optimal drug therapies for each patient including
pharmacokinetic dosing and monitoring services.
Establish a pharmacy practice specialty or an area of expertise and act as a resource for this specialty
providing education monitoring and other support pertaining to this specialty to improve the quality in the
department.
Documents and monitors all interventions and their outcomes.
Performs medication order entry in a manner consistent with current department policies and procedures.
Coordinates the daily distribution process of the pharmacy.
Represents the pharmacy in hospitalwide committees. Participates in pharmacy committees.
Provides written and/or in service education on a regular basis.
Performs other projects or duties as required or assigned.
