Pharmacy technician skilled in medication management for both inpatient and outpatient settings to help navigate discharge medications and postdischarge medication management from acute care facilities. Assist transitions of care pharmacists inpatient and ambulatory pharmacists with transitions of care management of recently discharged patients. Provide direct patient contact including post discharge follow up telephone calls and troubleshooting of prescription medication access. Performs pharmaceutical procedures for medication management that ensures safe effective and cost efficient medication therapy. Exercises sound judgment and developed clinical skills to provide pharmaceutical support to the patients plan of care based on the diagnosis in a timely manner. Adheres to all local/state/federal regulations codes policies and procedures to ensure privacy and safety. May also be responsible for performing specific procedures and/or orienting other staff to the department. Supports and participates in the overall operation of the pharmacy technicians including staffing and work assignments of the technicians. Serves as the liaison between the department and staff information systems physicians patients family and other site staff. Performs pharmaceutical procedures for medication preparation and distribution that ensures safe effective and costefficient medication therapy. Exercises sound judgment and has developed clinical skills to provide pharmaceutical support to the patients plan of care based on the diagnosis in a timely manner. Adheres to all local/state/federal regulations codes policies and procedures to ensure privacy and safety. May also be responsible for performing specific tasks and/or orient other staff to the department.Job Description:
