Job Description Summary
The MUSC Interstitial Lung Disease (ILD) Program is seeking a masters prepared nurse to help lead this dynamic team into the future. The center is a member of the Pulmonary Fibrosis Foundation Care Center Network and nationally recognized for its expertise in pulmonary fibrosis exceptional patient care and strong participation in novel therapeutic research. We are looking for an individual who is motivated to build relationships with both team members and patients to enhance clinic efficiency education community outreach and research to improve our patients outcomes.Entity

Job Description
The ILD nurse coordinator will participate as an expert practitioner in a variety of direct and indirect patient care activities that will enhance the overall quality of clinical care of the interstitial lung disease population. This position will directly report to the Program Manager but will have a matrix reporting structure that involves all applicable divisional faculty.
Job Duties:
30 Care Management: Responsible for having primary role in the coordination of care for patients with interstitial lung disease. Coordinates the care and services for complex Interstitial Lung Disease patients identified as needing assistance with care management / and or scheduling coordination coordinates outpatient therapies consults and treatment plans. Participates in the evaluation and treatment planning of patients in the Ambulatory setting focusing on new referrals but contributing to followup visits and hospitalized patients. Coordinates outpatient consultations therapies and treatment plans. Serves as an advisor for nursing staff when contacted by patients for management of medical problems. Identify resources available to support patient needs which includes providing information on community resources and facilitating patent access to basic needs. Serves as the voice of the patient across care settings and empower the patient to problemsolve options of care and promoting the patients right to selfdetermination. Attends participates and facilitates in patient meetings and conferences as appropriate. Assists with facilitating an evidencebased multidisciplinary and multispecialty approach to ILD management across the care continuum. Practices solid communication skills and is able to articulate and translate the patients condition to other care providers and to negotiate and make recommendations for changes in patient care.
30 Ambulatory Clinical: Works collaboratively with clinicians in navigating the patient needs (ranging from minimal to complex) from the diagnostic phase through treatment planning medical management and surveillance assuring the patient has appropriate timely medical and supportive services. Works with the program manager in the facilitation of all new referrals through an indepth medical review to determine pre appointment testing and consultations. Collaborates with central and pulmonary division scheduling to ensure efficient scheduling with appropriate provider. Oversight of clinic standardization of clinic orders for new referrals and return patients in collaboration with current resources. Reviews triages and provides support for ILD EPIC in basket for sick calls and patient inquiries and assists in issue resolution follow ups on labs and results and /or interpretation as directed by the managing physician. Performs med check / reviews / medication management including prescription refills monitoring for drug toxicity and prior authorizations ensures compliance patient safety and awareness. Initiates therapies ordered by the managing physician to include but not limited to outpatient infusion therapies and medications. Collects indepth information about a patients medical functional and social condition to identify individual needs in order to assist in the development of a plan to meet those needs. Leads preclinic PreVisit Planning meetings and assists team in determining needs for each patients clinic visit (e.g. annual labs changes in therapy vaccinations education). Works with patients to develop specific goals and actions based upon assessment. Initiates / facilitates / collaborates with all members of the interdisciplinary team to improve care delivered to the patient ensuring appropriate referrals (interdisciplinary and community) are made and completed.
20 Education: Enhances the quality of patients care through effective and efficient use of resources. Develop education resources for patients with diffuse lung disease in concert with the program manager and supervising physicians to empower patients and caregivers for improved quality of life and adherence to complex medical regimens. Develop materials to highlight our clinical research portfolio for both patients/caregivers and referring physicians. Serve as a resource to identify potential patients eligible for clinical trials and collaborate with MUSC research staff to offer opportunities to our patients develops materials to highlight the programs clinical research portfolio for both patients / caregivers and referring physicians. Promotes disease awareness through community educational activities in collaboration with the national and state programs attends conferences annually when available to maintain knowledge and competence as a nurse navigator. Serve as a resource to clinic nursing and CMA staff on common adverse events in our population and ensure standardized approach to the management of these issues in collaboration with the supervising physicians. Active participation within the Care Center Network of the Pulmonary Fibrosis Foundation including utilizing its education resources as well as engagement at annual meetings and foundation goals.
10 Quality Improvement:Leads clinical and quality improvement initiatives to improve the outcomes for ILD patients and families of MUSC. Monitors evaluates and identifies areas for process improvement. Takes personal responsibility in improving patient satisfaction with the quality of care and service. Participates in annual program updates for standard operating procedures and policies. Participates in QI activities and relevant research as designated by the ILD program manager and physicians. Collects data for evaluation to improve the ILD patient population.
10 Communication and Outreach: Coordinate the biweekly ILD multidisciplinary case conference among pathology radiology and pulmonary providers. Coordinate a minimum of quarterly patients support groups with a goal of increased frequency of these over time. Collaborate with MUSC marketing to enhance outreach through the state and region of our program.
Additional Job Description
Bachelors degree in nursing required and three years related nursing experience. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).
