Job Description
Vivo HealthStaff is recruiting for Primary Care Physicians in Stockton Modesto Merced and Indio California. These are fulltime permanent positions offering salary benefits and bonus opportunities.
We are seeking compassionate and experienced Primary Care Physicians to provide comprehensive primary care to medically complex and aging patients in a clinicbased setting. The physician will work collaboratively with an interdisciplinary team to ensure coordinated patientcentered care across home clinic and facilitybased environments.

Locations: Stockton Modesto Merced and Indio California

Schedule: FullTime Monday to Friday; rotating night and weekend call

Employment Type: Permanent

Salary: $250000 $280000/year bonus equity
Key Responsibilities:
-
Provide direct medical care including assessments diagnosis treatment planning and ongoing followup
-
Collaborate with an interdisciplinary team to develop and update patient care plans
-
Coordinate care transitions and discharge planning with hospital and skilled nursing facility staff

Oversee medical care for patients in nursing homes and during hospital admissions

Supervise and support nurse practitioners and other clinical staff

Participate in 24hour care planning and rotating call coverage
Requirements:
-
Medical degree from an accredited institution and completion of an ACGMEaccredited residency program

Board certification in Internal Medicine Family Medicine or Geriatrics (preferred)

Active unrestricted California medical license
-
Active DEA and California CDS certification
-
Eligible for Medicare and MediCal provider enrollment
-
Experience working with medically complex or elderly patient populations preferred
