Job Description

Job Summary:

We are seeking a compassionate and resourceful Social Worker to provide emotional support resource coordination and advocacy for individuals and families facing social emotional or medical challenges. The Social Worker will collaborate with interdisciplinary teams to improve overall wellbeing and connect patients to essential community and healthcare resources.

Key Responsibilities:
  • Conduct biopsychosocial assessments and develop care plans

  • Provide counseling and emotional support to patients and families

  • Assist clients in accessing community resources such as housing food financial aid and healthcare

  • Collaborate with physicians nurses case managers and therapists to ensure continuity of care

  • Coordinate discharge planning and followup services

  • Advocate for patient rights and access to services

  • Respond to crisis situations and provide interventions when necessary

  • Maintain accurate documentation and progress notes in the Electronic Health Record (EHR)

  • Comply with all federal state and facility regulations and policies

Qualifications:
  • Bachelors or Masters Degree in Social Work (BSW or MSW) from an accredited program

  • Current state license or certification as required (e.g. LSW LMSW LCSW LICSW)

  • Prior experience in a healthcare behavioral health or community setting preferred

  • Strong knowledge of social work ethics HIPAA regulations and patient confidentiality

Preferred Skills:
  • Excellent communication and interpersonal skills

  • Ability to work with diverse populations in various emotional states

  • Strong organizational and case management abilities

  • Cultural sensitivity and traumainformed care approach

  • Crisis intervention and deescalation skills

Work Environment:
  • Inpatient Outpatient CommunityBased Remote or Hybrid

  • Fastpaced and collaborative interdisciplinary team

  • May require occasional travel or home visits depending on role
