Job Description

Position Title

Population Health Care Coord

Broadmoor Campus

Position Summary / Career Interest:

The Population Health Care Coordinator provides support to JPC clinics for managing overall patient population needs by assisting with identification of care opportunities providing patient outreach participating in performance/ process improvement efforts and recommending areas of opportunity to support the JPC patient centered medical home efforts.

Responsibilities and Essential Job Functions
  • Enhances quality initiatives by assisting clinics in care opportunity outreach through a variety of methods.
  • Works with clinics to ensure the accurate and reliable deployment of processes and protocols designed to meet identified goals.
  • Participates in the clinics performance process improvement initiatives. May conduct chart reviews special audits report validation and document performance gains.
  • Monitors performance indicators and analyzes data patterns/trends to assist clinics in achieving quality improvement goals.
  • Ability to work collaboratively with practice managers and external stakeholders assist managers in gathering and preparing supporting documentation and respond to changes and problems as they arise.
  • Understand various quality programs impacting ambulatory practices today such as patient centered medical home requirements meaningful use and HEDIS measures for the various health plans.
  • Assist with the implementation of processes in order to meet the patient centered medical home requirements. Work with clinics to ensure ongoing compliance. Recommend any new reporting needs.
  • Utilize available electronic tools such as EMR population health databases and HIEs to complete quality/performance improvement initiatives.
  • Identifies and assists in the development of educational opportunities for practice staff to enhance medical home responsibilities and patient experience.
  • Maintains uptodate reference materials and files related to health plan requirements.
  • Must be able to perform the professional clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
  • Licensed Practical Nurse
  • High School Graduate

Required Licensure and Certification
  • Basic Cardiac Life Support (BLS or BCLS) American Heart Association (AHA)
