Location: Springfield MA
Schedule: MondayFriday 9a5p (with flexibility for program needs)
Base Salary: $50000$52000/year $1500 differential per additional case (see details below)
ServiceNets Adult Family Care Program is seeking a compassionate and dedicated Case Manager to oversee and coordinate care for individuals living in qualified family settings. In this vital role youll help ensure that every individual we support receives personcentered care in a safe supportive and nurturing home environment.
- Manage and oversee the support of individuals in Adult Family Care placements
- Coordinate and monitor services provided by Care Providers Primary Support Staff and Respite Providers
- Ensure all services meet agency standards MassHealth regulations and quality expectations
- Identify and respond to stressors and conflicts to maintain placement stability
- Provide crisis intervention services when needed
- Collaborate with MassHealth and other state/private agencies
- Complete required assessments reports and documentation in a timely and professional manner
- Supervise direct support staff and oversee care/respite provider performance
- Broad knowledge of human services or social work
- Bachelors degree in a related field required
- 35 years of progressively responsible experience
- Excellent organizational interpersonal and communication skills
- Working knowledge of computer systems and software (e.g. Microsoft Office)
- Valid drivers license and reliable transportation
- Base Salary: $50000$52000/year (based on experience education and workload)
- Differential: $1500 for each additional case above the base caseload of 26
- Caseload Range: Minimum 26 Maximum 30
