Job: Licensed Master Social Worker Classification: Hourly/NonExempt Job Category: 2 Professionals Position Summary The Licensed Master Social Worker coordinates the discharge planning and social work functions of the hospital and assesses patients social emotional and financial needs as they impact on their continued care.
Essential Functions Completes social work progress notes on assigned patients. Completes proper documentation to indicate instruction provided. Assures coordination of departmental duties on different nursing units while helping to assure patients with needs are identified. Assures that current records are maintained so as to allow other disciplines to be aware of the progress/needs of patients. Provides counseling and emotional reassurance to patients with psychosocial needs. Coordinates mental health services for patients with needs for inpatient mental health treatment. Discusses condition and treatment of patients with physicians/ other staff members to analyze patients psychosocial needs. Instructs patients and family members on community resources. Makes referrals as appropriate to meet the needs of patients and their families. Demonstrates an awareness and understanding of the patient and familys need for comfort and security. Assists patient/family in the completion of Advance Directives when requested. Assists supervisor/director with special projects when requested. Assures that patients are informed of support services available for their specific illness i.e. cancer diabetes stroke etc. Actively makes postdischarge arrangements for patients who have identified needs i.e. nursing home home health services hospice and DME. Assists the patient/family in applying for and obtaining appropriate financial assistance. Attends regularly scheduled utilization review/discharge planning meetings. Demonstrates effective oral and written communication skills. Demonstrates counseling skills. Adheres to and follows all patient experience initiatives. Reports to and departs from work on time and as scheduled. Performs other duties as assigned. Must be able to deal with stress while maintaining composure. Follows and adheres to TRMC vaccine policy(s) mandated by the Centers for Medicare & Medicaid Services (CMS).
Work Experience One year of social work experience in a hospital setting preferred.
Education LMSW Licensed by the Texas State Board of Social Workers required. BLS (Basic Life Support) within 30 days of hire date.
