Home Health Physical Therapist (PT)
A Home Health Physical Therapist is needed in the Bronx NY.
The Bronx offers a dynamic urban environment with diverse cultural experiences and easy access to public transportation. This position is with a missiondriven home health organization committed to delivering highquality personalized care to patients in their homes.
Salary and Benefits
- Salary range: $100000 $120000 annually
- 25 PTO days in year one

Responsibilities
- Provide inhome physical therapy following the physicians care plan
- Achieve 30 points per week treating 6 patients per day
- Assess treat and adjust individualized care plans to improve mobility and recovery
- Monitor patient progress document outcomes and adjust treatment as needed
- Educate family members and caregivers on rehabilitation techniques and patient care
- Collaborate with the care team to ensure comprehensive patient care
- OASIS experience preferred
Requirements
- Current Physical Therapy Licensure in New York State
- Graduate of an accredited PT program
- Proven experience in home health or physical therapy
- Strong communication and interpersonal skills
- Knowledge of current treatment practices and clinical expertise
