Overview
Medication History Pharmacy Technician
FullTime; FTE 1.0
MondayFriday; 8:00am4:30pm every 3rd weekend
Bettendorf IA
Under supervision of a licensed Pharmacist the Medication History Pharmacy Technician is responsible for completing the most accurate current medication list possible on newly admitted inpatients through information obtained from the patient family pharmacy physician office or other facility as required. Inputting correct home medication information dose frequency time of last dose primary pharmacy and allergy information into the electronic medical record. Has excellent knowledge of brand and generic medication names common dosages and dosage forms and frequency of administration of each medication.
Responsibilities
- Interview newly admitted patient or patients family to obtain the most accurate current medication list possible.
- Contact patients family nursing facility pharmacy and/or physician office for medication clarifications as needed.
- Obtain indications for PRN medications and actual frequency of use of PRN medication.
- Prompt patient for additional medications by specifically suggesting over the counter (OTC) medications including herbals vitamins pain relievers cold products patches lotions & creams eye drops sprays and inhalers.
- Determine if patient was on recent antibiotics prior to admission.
- Input most accurate current medication list possible into electronic health record.
- Update patients primary pharmacy and other pharmacies the patient may use in the electronic health record.
Qualifications
Education:
- High School Diploma or Equivalent
Experience:
- 2 years experience as a Pharmacy Technician
License(s)/Certification(s):
- Certified Pharmacy Technican (CPhT).
- Pharmacy Technician license or eligible licensure in both IA and IL.
- Most obtain secondary licensure within 90 days.
