Description

In coordination with the Director of Heart and Vascular Service and departmental leadership the Structural Heart Program Coordinator develops and maintains a comprehensive structural heart program with best practice/service quality and outcomes. Responsible for monitoring the process and clinical operations of a dedicated structural heart clinic structural heart multidisciplinary conference and all patient staff and other stakeholders. Provides education guidance and direction in the identification and ongoing surveillance of structural heart patients maintenance of relevant quality/patient databases assistance with national registries measurement of program outcomes and participation in process and quality improvement initiatives. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.

Job Functions:

  1. Collaborates with multidiscipline departments to coordinate all activities for the structural heart patient population (TAVR Mitra Clip LAAC ASD and PFO) across the continuum from identification through discharge and follow up.
  2. Coordinates valve clinic and structural heart conferences. Creates case presentations and necessary clinical documentation as needed.
  3. Collaborates with valve vendors and transmits data/images for review if necessary.
  4. Participates in quality improvement initiatives collects data monitors outcomes understands and utilizes the Performance Improvement process and makes recommendations for process improvement initiatives.
  5. Works closely with Structural APPs to ensure patient safety and that quality care is given in an efficient timeframe.
  6. Communicates effectively both written and verbally the progress of the program and identifies opportunities for growth and improvement to the medical administrative and cardiovascular quality care team on an ongoing basis.
  7. Develops and maintains program protocols policies and processes.
  8. Serves as a liaison and communication link between patients and their families with referring physicians primary care providers specialists and patient care services.
  9. Communicates with other hospital/medical group staff to manage the providers office and procedure schedules.
  10. Oversees and provides structural heart disease education to patient care staff (physician clinics inpatient units etc).
  11. Participates in marketing and outreach education initiatives both within and outside of the hospital.
  12. Participates/attends national local and internal educational conferences to enhance knowledge of structural heart disease.
  13. Assesses patient problem/needs by direct interaction with the patient and/or screening and review of the patients medical record.
  14. Provides patient/family needs assessment including physical psychosocial spiritual financial and educational.
  15. Participates in community events (health screenings educational fairs etc. to promote health awareness and educate the community on available services and support available at Memorial Health System.
  16. Assumes all other duties and responsibilities as necessary.

Qualifications

Minimum Education/Experience Required:

  • Bachelors Degree in a health related field required.
  • Licensed in the State of Ohio as a Registered Nurse required.
  • BLS certification required.
  • Minimum of 3 years of experience with cardiac nursing preferred.

Special Knowledge Skills Training:

  • Excellent verbal presentation and written communication skills.
  • Ability to get along well with a variety of personalities and individuals.
  • Demonstrated proficiency in computer and Microsoft office application skills.
  • Excellent skills in the following areas: critical thinking problem identification and resolution adoption to change and negotiating.
  • Organizational skills necessary to work independently to follow through on daily activities multitask and effectively manage time and resources.
  • Proactive and selfdirected with the ability to prioritize and make decisions in a fastpaced environment.
  • Analytical skills necessary to understand the relevance and impact of data and clinical information to support the appropriate utilization of resources associated with the cost and quality of patient care while keeping the focus on positive patient care outcomes.

Compensation Details: Education experience and tenure may be considered along with internal equity when job offers are extended.
