• This opportunity will see you work from home as well as visiting clients in their own home in and around the Ballarat region.

• You will be involved in planning implementing and evaluating services for clients to maintain & improve their health and independence
• You will conduct initial and ongoing and complex assessments involving direct client contact and clinical care
• As a part of the team youll assess &understand the needs of clients and ensure that the clients needs are translated into a personal plan
• You will be supported by the Podiatry team as well as multidisciplinary team to pursue opportunities for professional development.
