In either a clinical or a field setting interviews clients with problems such as personal and family adjustments finances employment food clothing housing and physical and mental impairments to determine nature and degree of need.
Gathers information such as medical psychological and social factors contributing to clients situation; evaluates these together with clients strengths.
Provides limited advice to clients individually in families or in other small groups regarding how to meet personal needs and aids clients to improve social functioning and life skills.
Participates in helping clients to modify attitudes and patterns of behavior by providing training and educational programs that help clients understand the nature and source of personal problems.
Refers clients to community resources and other organizations as indicated. May determine or participate in determination of client eligibility for financial assistance.
Compiles records prepares reports and develops and/or reviews service plan. Follows up to determine quantity and quality of service provided to the client and status of clients case.
Accesses and records client and community resource information either on paper or via electronic means to input and retrieve information.
