Role Description:

The Medical Director will be joining Oak Street Health at a critical time in the history of our companyand of primary care. The opportunity for impact is large and growing as Oak Street is leading thetransition from feeforservice to value basedbased care. The Medical Director will be responsiblefor leading as a dyad partner with the Mobile Practice Manager the inhome primary care provider teams in the specified geography.

The Oak Street Health At Home division is an interdisciplinary team that focuses on primary care and transitional care support for our highestneed patientsin their homes. The primary responsibility for the Medical Director is direct management oversight and coaching of the local team and is part of the At Home divisions leadership team which helps develop programs(i.e. complex care coordination chronic disease management psychosocial support etc.throughout the division. This entails collaboration with theclinical teams in the center medical directors and other partners outside of Oak Street Health. The MedicalDirector will see patients in their homes up to 3 days a week alongside their provider colleagues.Additionally we see this leader being a key thought partner in providing feedback on our clinicalmodel especially as it pertains to the At Home Program.

Core Responsibilities:

The Medical Directors primary duties are focused on the management of their teams. Key responsibilities include:

• Leading At Home providers and ensuring their priorities and outcomes are in line with those of the organization

• Providing timely and constructive feedback to providers so the medical group continues toimprove performance for patients and colleagues

• Helping providers master the datadriven tools and practices required to keep patients welland out of the hospital when avoidable

• Reviewing regular quality dashboards with providers coaching on tools to improve qualityoutcomes

• Coach providers on appropriate coding and documentation

• Provide managed care experience/teaching to providers

Additionally the Medical Director will be part of the At Home leadership team that will have strategic input into a variety of divisional projects that support theenhancement of the care model including:

• Medical management care management utilization management and quality improvement tools and processes

• Development of clinical compliance policies and guidelines

• Representing Oak Street to external partners (e.g. vendors payers providers) andcreating relationships and partnerships that support our mission and our valuebased model

Additional responsibilities include:

• Understanding and communicating provider performance/compensation plans and beingable to answer questions and deliver results to direct reports

• Serving as formal manager doing regular performance reviews andcheckins approving paidtimeoff requests and managing miscellaneous human resourcetopics for providers in the market

Our Medical Directors are expected to support this cultureand set an example for others to follow.

What are we looking forWere looking for motivated experienced Primary Care Providers with:

• A passion for the geriatric population and amotivation to be a part of a fastgrowing organization

• Geriatric certification and/or other professional degrees (e.g. M.B.A J.D. M.P.H) welcomebut certainly not required

• Experience with managed care and/or valuebased practice

• Experience using a metricsdriven approach to analyze cost quality and satisfaction data todrive clinical strategy and program redesign

• An excitement for developing and implementing new processes

• An understanding of the basics of managed care and a recognition that population healthand a focus on the smart allocation of scarce resources can lead to highvalue care (better carelower cost)

• An ability to remain calm in stressful situations

• A commitment to older adults and to lowincome patients

• Comfort with ambiguity and passion for problemsolving

• US work authorization
