Job Description

- Evaluate and reevaluate clients functional status per organizational bylaws and regulations.
- Develop and implement individualized treatment plans to address client needs.
- Treat 812 clients per day providing evidencebased physical therapy interventions.
- Monitor and document client progress ensuring compliance with regulatory standards.
- Communicate regularly with Program Directors nurses team members and staff regarding client updates changes and recommendations.
- Collaborate with rehab coordinators Occupational Therapists (OTs) and SpeechLanguage Pathologists (SLPs) to discuss client care and clinic operations.
- Coordinate with National Seating and Mobility representatives to schedule wheelchair appointments for clients.
- Complete and maintain accurate documentation in accordance with regulations and organizational policies.
- Prepare and submit letters of medical necessity for wheelchair modifications.
- Develop home exercise programs tailored to individual client needs.
- Distribute exercise programs to designated houses and ensure staff compliance.
- Education: Doctorate or Masters degree in Physical Therapy from an accredited program.
- Licensure: Must hold a current and valid Physical Therapy license in Massachusetts.
- Experience: Previous experience in a clinical or rehabilitative setting preferred.
- Skills: Strong communication collaboration and organizational skills.
- Knowledge: Familiarity with rehabilitation equipment and best practices in physical therapy treatment.
- Ability to work in a clinical setting with clients of varying cognitive and mobility levels.
- Must be able to lift/move patients as required for therapy.
- Travel between client locations may be required.
