In this innovative nursing position with Geisingers Hospital at Home youll bring healthcare to our community where they need it most right where they live. Geisingers athome care program helps keep patients healthier safer and better connected to their care team.
Youll work closely with primary care physicians Geisingers team of doctors dietitians case managers pharmacists and other medical support staff as you visit patients and provide care right in their homes.

RN Registered Nurse Hospital at Home Per Diem
- Hours:Per Diem as needed days 8:00AM to 6:30PM start time); flexibility in scheduling based on patient needs.
- Weekends:Up to every other weekend as needed
- Holidays:Occasional as needed
- On Call:Rotation based. Will be reviewed at the interview.
- This position is communitybased and requires travel throughout Montour and surrounding counties.
- Experience:Two 2 years of RN work experience is required.
- Preferred Experience:Acute care or inpatient nursing experience. Emergency medicine experience is helpful but not required.
Job Description:
- Collaborates with staff providers and brickandmortar hospital providers to identify patients and coordinate a seamless transition to the Acute Hospital Care at Home unit.
- Communicates daily potential capacity and operations with staff via team huddles.
- Coordinates patient care schedules diagnostics and delivery of durable medical equipment.
- Triages and manages acute patient needs and barriers by coordinating efforts of the care team.
- Provides direct care to patients in their home.
- Collaborates with doctors paramedics diagnostic services ancillary services and remote patient monitoring technologies to provide inhome care for patients with acute medical conditions.
- Provides patientcentered care to identify barriers and ensure that patient and family needs are met through coordinating efforts of the care team.
- Provides medicalsurgical level of care to patients in their homes.
- Works with other team members to respond to acute needs and prevent escalation to higher level of care.
- Ensures adherence to quality standards and participation in quality monitoring and improvement.
- Participates in ongoing case review for delays or variances in care; raises issues with appropriate department attending physician and physician designee.
- Uses approved medical necessity criteria as a tool to ensure that patients meet appropriateness of the setting.
