Concern Housing is seeking a Daily Living Skills Counselor who will be responsible for providing life skills training services directly to clients and is considered a resource in that they should be familiar with the skills and tasks necessary to help consumers learn self reliance skills. The primary task is to provide hands on assistance to clients to help them to develop the skills necessary to maintain independence in the supported housing program.

Position: Daily Living Skills Counselor
Location: Middle Island NY
Schedule: Monday Friday; 9am 5pm
Salary: $41600/yr.

Responsibilities include but are not limited to:

Adherence to all policies as set forth in Concerns personnel manual.
Provide education and skill building assistance across multiple activities of daily living including but not limited to
maintaining personal hygiene maintenance of the living environment laundering of clothing travel training money
management food preparation nutrition shopping basic self preservation skills communication skills community
integration and wellness selfmanagement.
Provide individual counseling and support services as it pertains to life skills development.
Work as an integral part of the case management team. Maintain open communication with the case manager regarding
skill assessment and goal planning activities.
Develop training tools and techniques in coordination with case manager to address identified client needs.
Maintain all documentation pertaining to visits and activities as consistent with agency policy.
Conduct site visits as needed and directed by Supervisor.
Attendance at weekly supervision and staff meetings.
Remain current with all Agency required compliance documents and trainings.

Requirements: Three years experience working in a similar setting/and or working with people with mental illness. Bachelors degree in Psychology Rehabilitation Social Work or other related mental health discipline preferred. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
