LOCATION:Santa Monica California
***Full Time 12:00pm8:30pm shift.
SALARY: $94972 to $150321
$10K Sign On Bonus

SCREENING QUESTIONS:

- Do you have 2 years acute care inpatient experience specific to utilization review care coordination & discharge planning

JOB DESCRIPTION:
Lead Care Manager LCSW at our Health Center located in Santa Monica CA.
NOTE: The position available is for Full Time 12:00pm8:30pm shift.
- Utilize your transferrable skills to assess and coordinate care for a diverse population of patients in the community.
- Collaborate with our Multidisciplinary Team as well as patients and their families to ensure safe high quality and effective delivery of care.
- Case Management is a collaborative practice including patients caregivers nurses social workers physicians payers support staff other practitioners and the community.
- The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management.
- Recognizing the patients right to selfdetermination the significance of the social determinants of health and the complexities of care (barriers to coping abuse mental health substance use lack of resources psychosocial issues etc..
REQUIRED QUALIFICATIONS:
- Masters Degree in Social Work from an accredited program.
- Upon hire: California Clinical Social Worker License
- 2 years of experience in healthcare related field (Acute Ambulatory or PostAcute)
- 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
