As we continue to grow our team we are looking for Case Managers to join our passionate dedicated and BRAVE team! If you are a collaborative innovative individual who is interested in helping people recover from a mental health barrier increase their independence and find housing in York Region and South Simcoe than this role is for you!

Permanent: We are hiring for 2 permanent fulltime positions 35 hours per week; evening hours and weekends may be required.
Salary: $56444 per annum.
Benefits: Health & Dental Healthcare of Ontario Pension Plan (HOOPP). Generous paid time off including vacation sick and personal days.
Location:
The successful candidate be required to work out of the Aurora Office located at 15150 Yonge Street Aurora Ontario and will be required to travel extensively within York Region & South Simcoe for client meetings staff training and meetings.
Job Profile:
The Case Manager works within a Housing First model and collaborates with clients to provide individualized and flexible support to help them find housing engage and integrate them into the community helps to facilitate their access to resources and supports to maximize their independence and potential and promote their recovery. The work with clients is achieved with a focus on Housing stabilization.
Individuals are assisted to identify strengths and needs and develop long term goals short term objectives to meet these goals and concrete action steps to achieve the objectives. In addition the Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability. The Case Manager may be assigned other duties as required.
In addition to compliance with all organizational policies and procedures the incumbent is also expected to behave ethically demonstrate competence in effective communication and team building demonstrate effectiveness in working in a collaborative environment and demonstrate confidence in decisionmaking and in building relationships.
The Case Manager may be assigned to different programs as required based on need (i.e.: Flexible Support ACTT Case Management etc)
