Social Worker / LMSW or LCSW / Care Transition Manager
Work location:Texas Health Arlington Memorial800 W. Randol Mill Road Arlington TX 76012
Work hours:Full Time Days Monday Friday; 40 hours per week

Texas Health Arlington Memorial is a Joint Commissioncertified Primary Stroke Center a Chest PainCenter and a Heart Failure Center. Weve been recognized by the American Heart Association with a Gold Plus award for the exceptional care we provide to our stroke heart failure and chest pain patients. What Youll Need
- Masters Degree in Social Work is required.(Individuals hired as a CTSW prior to May 11 2017 will be grandfathered to the CTSW position with BSW at the entity they were employed at on May 11 2017.
- 3 years ofacute care hospital/medical social work is highly preferred.
- 1 year of discharge planning and/or care management is highly preferred.
- LMSW Licensed Master Social Worker or
LCSW Licensed Clinical Social Worker is required upon hire. - ACM Accredited Case Manager or
CCM Certified Case Manager upon hire is preferred. - ANCC upon hire is preferred.
- CPR Cardiopulmonary Resuscitation is required upon hire.
What Youll Do
- Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner.
- Reviews the Texas Health Readmission Indicator List (THRIL) scores daily for all assigned patients and collaborates with the interdisciplinary team to identify high risk patients whose THRIL score may not have indicated appropriately.
- Promotes discussion and assists in the identification of a primary care physician (PCP) for patients without a PCP.
Serves as a content expert on the following:
- Compliance requirements for delivery of HINN second IMM and MOON letters
- Potential denials avoidable days and alternate level of care day
- Medical necessity patient status and discharge criteria
