Responsible for completing psychosocial assessments and coordinating the interdisciplinary patient care plan for mental and behavioral health patients. Acts as a liaison between a patient or family and health care personnel to ensure necessary care is provided promptly and effectively. Works with internal and external resources to coordinate a timely and safe transition of patients to the appropriate level of care.
Evaluations diagnosis and treatment
Perform psychosocial and diagnostic assessments
Diagnose patients in accordance with the current DSM
Involve collateral sources in the patients treatment as clinically indicated (e.g. family friends etc.
Provide individual couples family and group psychotherapy services as indicated by the treatment plan
Provide crisis psychotherapy interventions
Provide substance initial screenings or substancerelated crisis intervention
Refer patients to other services as clinically indicated
Willingness to work with all patients seen at ones site of care as indicated by the site. (i.e. ages diagnostic presentations comorbidities etc. in accordance with ones scope of licensure and abilities
Utilize evidencebased practices in the treatment of patients.
Follow all ethical and regulatory guidelines
Provides culturally and linguistically appropriate services
Treatment planning discharge planning and referrals
Keep patient family or others informed as to the patients progress throughout treatment as the patient desires.
Complete and update the safety plan
Provides support and supervision for psychoeducational and peer lead groups
Maintain timely and accurate documentation
Make necessary referrals
Provide patient education relating to postacute levels of care/services
Assure patients SDOH needs are met for an effective treatment plan
Coordinate pharmacy and outpatient services followup
Assure that the patient has a 7 day followup postdischarge
Align transportation and follow up with natural supports on behalf of the patient
Collaboration and coordination
Participate in all required training and educational activities
Participate in departmentlevel meetings and huddles
Be responsive to email phone messages and patient portal messages
Ensures CCBHC standards are being meet within the area of the department
Education:
Masters degree in Social Work or Counseling Master of Science Degree in Social Work required
Professional Counseling or Marriage and Family Therapy with a clinical/mental health concentration from an accredited school preferred
Experience:
2 years relevant social work and/or counseling experience in a health care setting required
License(s)/Certification(s):
LMSW LISW LMHC or LMFT required
May be required to obtain BLS within six months of hire for certain sites.
Knowledge/Skills/Abilities:
Excellent communication skills knowledge and experience in good interviewing strategies. Demonstrate knowledge skill and ability to provide services to child adolescent adult and/or geriatric population. Individual will demonstrate effective written and verbal skills allowing communication with physicians coworkers other clinic/hospital personnel as well as the public. Position requires good organizational skills.
