About the Role
As a Specialist Navigator you will offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following a discharge from hospital. Providing intensive one to one support to individuals to access statutory assessment and supporting the development of a holistic and person centred strengthbased personalised plan according to the needs of the individual.
You will work as a single point of contact connecting the Nottinghamshire Healthcare Trust Nottingham City Councils Housing Solutions service and accommodation providers to assist with the process of acquiring and then resettlement into accommodation following discharge from hospital.

This post is covered by the statutory duty under Part 7 of the Immigration Act 2016 (English Language Requirement for public sector workers) and therefore the ability to speak fluent English is an essential requirement for the role.
A DBS enhanced check for a regulated activity is required for this post.
