Position Summary: Manages a case load of antenatal postpartum newborn and pediatric home care patients. The nurse will complete the nursing process; assessment diagnosis planning implementation and evaluation for patients in the case load using a familycentered and developmentally appropriate approach. Collaborates with members of the interdisciplinary team health care providers and community services to ensure that patient needs are met and utilization of services is effective. Supervises other home care staff as necessary.
Principal Duties and Responsibilities:
Develops a plan of care under the direction of the patients physician and in collaboration with the patient their family and/or the primary care givers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate. Initiates referrals for interdisciplinary services and communicates with other disciplines in a timely manner.
Provides ongoing assessment of the patients clinical functional and mental status to identify problems and/or progress toward desired goals. Communicates significant changes to the patients physician and clinical manager as appropriate. Identifies and communicates concerns regarding patient safety and/or environmental safety issues to the management staff. Follows the agency policy when necessary mandatory reporting is required.
Completes and submits accurate and relevant clinical notes and other required documentation in a timely manner in accordance with expected agency standards i.e. admission papers clinical notes status change verbal orders/memos; payer source documentation including authorizations discharge.
Supervises ancillary personnel (Licensed Practical Nurses Licensed Nursing Assistants) and delegates responsibilities when required. Establishes a care plan for paraprofessional services and revises as needed according to agency policy.
Completes nursing interventions and education relevant to the client. Reinforces caregiver and patient education provided by other health care providers. Arranges to have needed supplies and equipment in the patients home upon assessment of need.
Accepts responsibility of assignment to perform a specialized procedure and/or see patients in other programs as needed such as but not limited to Infusion Therapy and Hospice precepting of students and new staff etc.
Participates in the agencies after hours call and weekend visit program as appropriate in a responsible and willing fashion. Follows up on patient concerns appropriately.
Seeks opportunities for lifelong learning (journals outside educational programs grand rounds agency lecture opportunities etc. Maintains competency in skills for the Maternal Child Health program.
Demonstrates professionalism in all aspects of work.
