Work primarily from your preferred location in this remote role. Onsite presence is required only for compliance activities caregiver education sessions or skill development and quality assurance reviews etc.
- Conduct comprehensive nonmedical needs assessments using our structured assessment packages
- Develop and document detailed personalized nonmedical care plans tailored to each clients unique needs
- Perform regularly scheduled quality assurance visits of client care
- Generate leads and/or assist with converting leads to clients; while participating in business development activities to grow the client base to achieve monthly client conversion goals
- Provide care coordination services between clients families and their existing healthcare providers
- Be available for telephone consultation during agreed parameters.
- Provide caregiver education sessions or skill development
- Current unrestricted RN license in North Carolina
- Minimum of 2 years of nursing experience (home care experience a plus not required)
- Passion for supporting seniors and adults with diverse care needs
- Strong assessment and documentation skills
- Excellent communication and interpersonal abilities
- Demonstrated ability to work independently and manage time effectively
- Reliable transportation to travel throughout the Triangle service area
- Ability to provide your own tools/equipment
