Position Details:
Join a thriving academic primary care team based within Hartford Hospital providing outpatient care to a diverse community patient population. This position offers a unique blend of direct patient care and resident precepting within both the Primary Care Clinic and the Care Transition Clinic with flexibility in how you divide your time. Youll be part of a seasoned team of seven providers contributing to a dynamic teaching environment and supporting the next generation of clinicians.
Role: Open to Nurse Practitioners (FNP) and Physician Assistants (PA)
Location: 80 Seymour Street Hartford CT
Setting: 100 Outpatient Primary Care Adult Medicine 18
Schedule: Fulltime weekdays only!
Call schedule: No call required

Nurse Practitioners and Physician Assistants Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s) Nurse Practitioners and Physician Assistants perform routine physical examinations and formulate medical educational and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs e.g. referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the providers practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.
