Job Summary
Under minimal supervision plans organizes and implements social work treatment for clients and families. In addition develops and implements community services. Assesses the psychological needs of patients and families. Provides intervention to meet those needs utilizing personal family and community resources. Provides crisis intervention brief therapy case consultation and collaboration services.
Essential Functions:
- Conducts comprehensive biopsychosocial assessments that are thorough individualized objective and complete. Provides services appropriate to the presenting problem. Evaluates emotional components impact of health issues and assists client to identify strengths dysfunctional factors and to establish detailed and coordinated treatment plans. Applies knowledge of individual group and family functioning to ensure appropriateness of treatment methods to assist client in achieving treatment goals. Is aware of and able to articulate the professional use of self in the relationship to maximize clients adjustment enhance strengths and promote resolution of identified problems. Demonstrates ability to develop effective therapeutic interventions that are socioculturally sensitive. 25
- Routinely consults negotiates coordinates with internal and external resources to assure collaborative efforts to maximize patient outcomes. Demonstrates leadership in facilitating multidisciplinary communications and patient care meetings (i.e. care conference & rounds) and utilizes information to assess and reassess care needs. Demonstrates and educates staff as to the scope and function of the social work role in the health care team. Ensures patient involvement in goal setting with informed consent documented. Complies with applicable laws and educates other staff as to legal issues related to social work practice and/or issues specific to the unit/service. Prepares written documentation as required. Applies written guidelines to assure work is accurate and complete. Prepares and submits data and information to CSW in a timely manner. Charting is legible thorough timely and content reflects established qualitative standards. Activity codes and time allotments are supported by the content of patient record charting. Reports are filed in a timely manner to assure inclusion in statistical reports. 25
- Demonstrates knowledge and applicability of the principles of growth and development over the life span as well as demonstrating the ability to assess data reflecting the patients status and interpreting appropriate cultural information of the patient(s) to whom care/ services is being delivered/provided. Demonstrates knowledge of major concepts principles and theories of normal and abnormal human development its application to specific age groups and establishes a working differential diagnosis. Utilizes knowledge of patient populations cultural religious ethnic and social systems in interactions care planning and education. Utilizes working knowledge of medical terms abbreviations diagnosis treatment and prognosis and demonstrates this knowledge verbally and in writing. Demonstrates comprehensive effectiveness knowledge of programs eligibility and skill in obtaining appropriate resource/services; is able to educate others to identify gaps and suggest alternatives. Educates patients and their systems regarding risk factors for psychosocial functioning including but not limited to wellness and health promotion. 25
- Demonstrates willingness to enter into learning in the utilization of supervision. Recognizes growth needs and seeks opportunities to enhance learning/skills. Demonstrates ability to manage time and achieve established goals. Advocates as appropriate in a professional manner. 10
- Participates in learning of self and others through active participation in peer review process. Participates in staff meetings group supervision (staff development) and assigned task groups and applies knowledge of groups and their tasks through appropriate participation. 5
- Provides orientation reviews new employee manual and educates as to rules and standards effecting performance prior to intern providing care. Monitors reviews and cosigns all documentation to assure compliance with CSW qualitative and timeliness standards. Utilizes supervision to address learning needs concerns and practice issues. Establish plan to address issues. 10
- Promotes positive interpersonal (customer) relationships with fellow employees physicians patients and visitors. Treats these individuals with courtesy dignity empathy and respect; consistently displays courteous and respectful verbal and nonverbal communications. 0

Education:
Work Experience:
- Typically two years of full time post Masters Degree social work practice under the supervision of a licensed or equivalently qualified social worker or other DORA approved professional practitioner.
Licenses:
- LCSWLicensed Clinical Social Worker LCSW State Licensing Board required or
- LPCLicensed Professional Counselor LPC State Licensing Board required.
Knowledge Skills and Abilities:
- Knowledge of crisis intervention and brief and long term therapy processes.
- Knowledge of patient populations cultural religious ethnic and social systems in interactions care planning and education.
- Knowledge of medical terms abbreviations diagnosis treatment and prognosis and demonstrates this knowledge verbally and in writing.
- Skill in providing care appropriate to the age of the patients served within the unit.
- Skill in establishing a treatment alliance and engaging the client system in goal setting and prioritizing.
- Ability to manage the professional relationship including termination.
- Ability to respond to trauma or psychiatric emergency.
