Responsibilities of a Licensed Practical Nurse include:
- Performs prescribed individualized patient care to patients as delegated.
- Participates in patient case conferences as appropriate.
- Assists the RN in the preparation evaluation and modification of the patients plan of care as appropriate.
- Observes and reports to the RN the patients physical and emotional needs and reactions to treatment and to medications.
- Provides nursing care in a way that considers patients privacy comfort and safety.
- Demonstrates the ability to instruct patients in the home and at community programs.
- Performs procedures based on the Agencys policies and procedures.
- Performs selected procedures based on the Agencys competency testing.
- Administers medication after successful completion of a medication competency.
- Accepts verbal orders from physicians.
- Assists the RN in planned patient teaching.
- Documents patient information in a clear legible and accurate manner according to policy.
- Documentation is completed within established time frames to meet Federal and State requirements.
Qualifications of a Licensed Practical Nurse include:
- Must possess an active LPN license in both VT and NH.
- 12 years experience in home care preferred.
- Ability to communicate clearly verbally and in writing.
- Flexible in adapting to work schedule changes in order to meet patients 24 hour needs.
