About the Mental Health team
An exciting opportunity has arisen in the mental health social work service. We are seeking a highly skilled and confident lead practitioner with thorough knowledge of the care act mental health act and other key adult social care legislation. The role consists of leading the mental health discharge pathway by attending escalation meetings discharge planning meetings supervising dedicated hospital social care workers and working effectively with external and internal key professionals and services. Managing a small caseload may also be required. Knowledge of communitybased & preventative services is vital in contributing towards strengthsbased approach. As a mental health lead practitioner you will be responsible for developing effective ways of working in collaboration with health partners across the North London Foundation trust and community mental health teams. The role also entails providing highquality practice leadership supervision and case management support to social workers and AEOs within the pathway. To facilitate timely and safe discharges with all key partners it is imperative that close relations are built with the network navigators who coordinate discharges front line on the respective mental health wards the network navigators are employed by North London Partnership.

About the role
- Support the Team Manager to provide clear and responsive practice leadership and supervision and promote the highest professional standards in social care practice.
- Promote a culture of continuous learning and professional development of staff and students across Communities Adults and Health to enhance the quality of practice and staff wellbeing.
- To support people to maintain their health wellbeing and independence by providing access to good quality information and advice and signposting to local preventative services with a focus on building their resilience and strengths to stay well and achieve the outcomes that are important to them.
- To provide a high standard of social work to adults and/or carers arising out of the Councils duties and powers under the Care Act 2014 and in accordance with Barnet policies and procedures guidelines and the Code of Practice for social care workers.
- To provide appropriate professional social work assessments with adults and carers with a focus on meeting the social care outcomes as articulated in the Department of Healths Adult Social Care Outcomes Framework.
- To safeguard adults and carers at risk of harm and promote Making Safeguarding Personal principles and their wellbeing.
- To undertake and review individualised and outcomesled care and support. To ensure that the wellbeing principles are at the heart of assessment and care and support planning.
About you
- Has relevant social work qualification and registered with Social Work England
- Fully competent to Senior Social Worker level under the Social Worker National Competency Framework
- Has relevant social work qualification and registered with Social Work England
- Work collaboratively with partners maximizing joint working leading in agreed areas of work for both Social Care and the integrated care board.
- Provide professional supervision and mentoring to employees supporting caseload management. Promote reflective practice and continuous professional development.
- Participate in the completion of the decision support tool applying sound judgment based on the evidence obtained. Complete and contribute to care act assessments reviews and reassessments working within the legal context local policies and procedures.
