Job Description

The Nurse Navigator will guide patients and families through the healthcare system enhancing patient care and streamlining the treatment process for individuals with pulmonary conditions. In an outpatient pulmonology clinic the Nurse Navigator will serve as a single point of contact for patients and families by coordinating care and helping to ensure that their needs are met in a timely and efficient manner. As a member of the multidisciplinary team the nurse navigator will collaborate with other healthcare professionals to provide personalized care that is tailored to the individual patients needs and serve as an advocate and educator for pulmonary patients. Provides comprehensive support to patients with pulmonary conditions guiding them through their healthcare journey from diagnosis to treatment and followup care. Their primary function is to ensure the patient remains compliant to the treatment plan by building relationships with patients and physicians coordinating the plan of care assisting with appointments transportation needs education resource provisions and/or representation within the multidisciplinary care environment. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient highquality outcomes for pulmonology patients who may receive a cancer diagnosis including support with interdisciplinary and cross facility coverage and collaboration.

MAJOR RESPONSIBILITIES

  • Assess patients medical social psychosocial and other care needs using appropriate tools to patients needs and barriers to care and provide access to potential resolutions. Identify health disparities and assist in removal of these disparities. Respond to patient challenges/barriers to care until resolution is achieved.
  • Serves as a liaison between the patients and family all physicians involved in the patients care internal and external healthcare providers support network members and the wider healthcare community. Ensure smooth transitions between care modalities facilities and providers. Introduce patients to appropriate caregivers as needed. Connect patients to hospital and community resources.
  • Streamline processes for patients by assisting with appointment scheduling and paperwork preparation. Ensure the organization of appointments and explain the sequence of treatments.
  • Facilitate patient movement through the appropriate clinical pathway and collaborate with physicians to ensure patient compliance and empowerment to manage his/her own healthcare needs. Conduct follow up conversations with all patients and communicate any concerns changes or social needs to the appropriate physician or other care provider. Bills for services assisting the patient during all hours of the day.
  • Support practitioners to assist patients in understanding their diagnosis treatment options and the resources available including education clinic research studies and technological advances.
  • Provide appropriate teaching outreach and support to patients and families and comprehensive education to patients regarding their pulmonary conditions treatment options and selfmanagement strategies to promote adherence to care plans. Education/connect to resources on subjects that fall beyond the scope of individual modalities (access to supportive care financial support return to work).
  • Develop educational materials to various programs/procedures specifically to the Incidental Lung Nodule Program and Bronchoscopic Lung Volume Reduction Program.
  • Facilitate the evaluation and management of patients with incidental lung nodules by coordinating followup imaging consultations and necessary interventions. Educate patients about the significance of lung nodules potential risks early detections and the importance of followup care. Facilitates referrals and further management of care in the case of a cancer diagnosis.
  • Guide patients through the Bronchoscopic Lung Volume Reduction process providing education on eligibility procedures and expected outcomes.
  • Collaborate with the multidisciplinary team to assess patients for suitability for the Zephyr Valve procedure. Maintain accurate records of patient assessments procedure scheduling and outcomes related to the Zephyr Valve implantation. Monitor and track patient progress postprocedure addressing any complications or concerns and facilitating followup appointments as needed.
  • Ensure appropriate patient data are available and patients are appropriately assessed and documented upon including identification of appropriate clinical research study options.
  • Collaborates with the Executive Director of Clinical Operations to determine successes and opportunities related to monthly navigation scorecard reports. Make appropriate recommendations for changes to the current program both locally and at a corporate level and assist in delivering program improvement.
  • Maintain CEUs every year centered around Nurse Navigation.
  • Functions effectively in a Matrix Management environment.
  • Completes any duties and special assignments as requested.

EXPERIENCE EDUCATION AND CERTIFICATIONS

  • Minimum of three 3 years of experience as a Nurse Navigator in Pulmonary Nursing Care Oncology Nursing and/or related field required; experience working in intensive care (ICU) preferred
  • Bachelors Degree in Nursing required
  • Masters Degree in Nursing preferred
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) Certification required/preferred
  • Registered Nurse (RN) License required
  • Oncology Nurse Navigator Certification preferred

KNOWLEDGE SKILLS AND ABILITIES QUALIFICATIONS

  • Advanced knowledge of healthcare systems and regulations
  • Advanced knowledge of medical terminology and procedures
  • Advanced knowledge of Nevada Medicaid rationales for case management billing insurance billing and insurance practices
  • Advanced knowledge and development of a written plan of care
  • General knowledge of business principles involved in planning resource allocations human resource concepts operations policies and procedures
  • Strong leadership and management skills to promote teamwork and excellent customer service practices.
  • Demonstrated proficiency in Microsoft Office (Word Excel etc. and Google Suite (Gmail Docs etc)
  • Strong critical thinking problem solving and organizational skills
  • Excellent verbal and written communication skills
  • Must be able to work independently with minimal supervision
  • Must be able to multitask and prioritize work in a fast paced environment
  • Must be able to maintain confidentiality
  • Must be able to pay close attention to details

PHYSICAL REQUIREMENTS
