POSITION DESCRIPTION POSITION: Physical Therapist / Occupational Therapist

LOCATION: OSI Physical Therapy Clinics Forest Lake MN

REPORTS TO: Clinic Director


PRIMARY PURPOSE: Primarily responsible for providing patient care including evaluation of the patient developing a treatment plan applying physical agents/modalities and manual treatments and communicating with referral sources as appropriate to optimize the patients continuum of care. May also supervise the delivery of care by Physical Therapist Assistants Clinic Services Support Staff and Athletic Trainers when necessary.


RESPONSIBILITIES Identifies and leads opportunities to create an outstanding patient experience in the life cycle of a patient at OSI. Performs the initial evaluation of the patient requiring review of the initial patient information a subjective interview and observation and measurement of the client at rest and during movement to document the status of the patient. Develops the treatment plan and updates this treatment plan as required. Supervises the implementation of the treatment plan by appropriately delegating tasks to the clinical service support staff physical therapist assistants and athletic trainers. Applies treatments to the patient including the use of physical agents/modalities manual treatment procedures exercise instruction/progression and education of the patient and the patients family as required. As required supervises the training of clinical services support staff physical therapist assistants athletic trainers and student physical/occupational therapists. Accurately documents and inputs the medical progression of the patient into the patients medical record. Meets payor requirements for reimbursement and appropriately charges for rendered therapy services. Demonstrates support for the team and engages in appropriate teamwork. Performs activities and responsibilities in ways that assists others in doing their job successfully. Responsibly completes continuing education requirements to maintain physical and/or occupational therapy licensure. Additional responsibilities may include training and education of those not affiliated with OSI building relationships with various referral sources attending and participating in various staff and allcompany meetings participating and leading continuing education efforts provided by OSI.

Requirements

We are looking for a full time Physical Therapist to work out of our Forest Lake MN clinic with the possibility of working at any of our 8 clinics in the east metro area. MN Physical Therapist license required. OSI is willing to reimburse the expense of the license.

  • Ability to work at multiple locations within MN
  • Willingness to obtain WI license
  • Masters degree preferred
  • Pelvic Health certification or willingness to obtain (not required)
  • Physical & Rehabilitation Medicine
  • CPR certified preferred
