Hospital Navigator

Job Summary
The Hospital Navigator position is a colocated position stationed at John C. Lincoln Hospital. The position is responsible for engaging individuals with an opioid use disorder enhancing the likelihood of success through connection to Medication Assisted Treatment Outpatient Counseling and address Social Determinants of Health. The Hospital Navigator is an individual with lived experience who can use their experience strength and hope to model recovery instill hope and share a universal message of recovery in patients struggling with substance use/opioid use disorder. The Hospital Navigator will identify and reduce barriers for patients and connect them with needed services and resources in the community. This position works with patients who need additional support and engagement and will be responsible for coordinating outreach engagement case management and ongoing wraparound supportive services post hospital discharge with navigation support teams (CCHP SOR etc.. The Hospital Navigator regularly interfaces with hospital staff doctors RNs and social workers and participates in staffing as needed or requested.
Skills/Requirements
Highschool diploma or GED and either:
o 18 credit hours of posthigh school education in a field related to behavioral health and two years of fulltime behavioral health work experience; or
o four years of fulltime behavioral health work experience.
Has an associates degree and at least two years of fulltime behavioral health work experience; or
Has a bachelors degree in a field other than behavioral health and at least one year of fulltime behavioral health work experience; or
Has a masters degree or bachelors degree in a field related to behavioral health.
Minimum of six months of recovery from substance use and/or mental health disorders required. Experience as a CCHP Navigator preferred.
Experience in working in a hospital setting or interfacing with medical personnel preferred.
Current AZ Drivers License (valid and in good standing).
Clear 39month Motor Vehicle Record.
Behavioral Health Technician (BHT) in accordance with A.A.C. R910101.33 and CBI Clinical Policies and Procedures required.
Emergency Medical Technician certification (valid within the state of Arizona) preferred.
Certified Peer Support required.
